Patients: Getting started with IBD Connect

Getting started is simple. Follow these steps to download the app, link your account and begin using IBD Connect.

01. Download the app

Download IBD Connect from the Apple App Store or Google Play Store. Your treating team may also provide a QR code to help you find the app.

02. Sign in and link your account

Sign in using your Apple ID or Google account, then follow the prompts to link IBD Connect to your CCCare patient record.

You'll need to enter details that match your CCCare record and access the linking code sent to your registered email address.

03. Start using IBD Connect

Begin completing regular symptom and quality of life check-ins. You can also complete pre-clinic questionnaires when requested by your treating team.

Regular check-ins, even when you're feeling well, help you build a clearer picture of your health over time.

Frequently

Asked
Questions
Why won't my account link?

Your details may not match those recorded in CCCare. Check that your first name, surname, date of birth and NHI or Medicare number are correct, then try again. Also check your junk or spam folder for the email containing your linking code. If you still cannot link your account, contact your care team for assistance.

Does the email I use for my IBD Connect account need to match the email on file in my CCCare patient record?

No. You can sign in using your Google account or Apple ID, even if it uses a different email address. Your linking code will be sent to the email address on file in your CCCare patient record. If you no longer have access to that email, contact your care team to update in CCCare.

How will I know when I have a questionnaire to complete?

When your treating team requests a questionnaire, a card with orange text will appear under To do on your Home screen. Tap the card to open and complete your questionnaire before your next appointment.

Who can see my personal information?

If you link your IBD Connect account to CCCare, only your pre-clinic questionnaire responses are shared with your care team. At this stage, your personal symptom tracking, triggers, and IBD Disk results remain private and are not shared.

Future app updates may allow this information to be shared with your care team with your consent. If this happens, you will be notified and able to choose whether you share or not. If you choose not to link your IBD Connect account to CCCare, no information is shared with your care team.

The app isn't working properly. What should I do?

Try closing and reopening the app, checking your internet connection and installing any available app updates. If the problem continues, submit feedback through the app. If the app will not open, contact IBD Connect Support at CCCure and share your phone model and a description of the problem. Please do not include medical information.

Clinicians: Supporting your patients with IBD Connect

Introducing IBD Connect to your patients is straightforward and works alongside your existing CCCare workflow. Follow these five steps to help your patients get started and make the most of the app.

01. Agree on Your Site Strategy

We suggest that you target people with upcoming review appointments, perhaps within the next 2 to 6 weeks as their pre-clinic questionnaire is a good trigger to download, link and use the app.

You may wish to start with people well known to the service, and perhaps include those attending for routine PBS review encounters.

02. Communicate with Your Consumers

It’s up to you how you’d like to share information about IBD Connect with your consumers. Options include a newsletter, email, QR code in the out-patient department.

Share the patient information resources above to help people get started.

03. Monitor for Successful Downloads & Connections

Monitor for people successfully downloading IBD Connect and linking to Crohn's Colitis Care (CCCare).

04. Sending the first Questionnaire via IBD Connect

To start getting the best value out of IBD Connect, you are now ready to send the first questionnaire. This will allow your patients to update their own information, complete their pre-clinic questionnaire, and receive reminders about blood and stool tests, all in advance of their upcoming appointment.

Remember to schedule the next follow-up when creating a Management Plan as this will trigger the pre-clinic questionnaire being sent to each person 2-weeks ahead of the next desired review date.

05. Continue Supporting Your Patient

Once linked, patients can:

  • Complete pre-clinic questionnaires
  • Record symptom self-assessments
  • Complete IBD Disk quality of life assessments
  • Track factors that may influence their condition

Encourage your consumers to schedule regular 2-minute check-ins, even when they're feeling well. These should be at an interval that works for them - whether that be weekly, fortnightly, monthly, or even quarterly.

Encourage patients to share feedback directly in app to help improve future development of IBD Connect. To submit feedback in app, scroll to the bottom of the Home page.

Clinician Support

Frequently

Asked
Questions
Do I need to install any new software?

No. People with IBD use the IBD Connect mobile app, while clinicians continue working within the Crohn's Colitis Care (CCCare) platform.

What information will I see?

With initial IBD Connect functionality, you will receive:

- Pre-clinic questionnaire responses

You will review these through the Questionnaire tab in CCCare, via the left-hand menu, as usual.

A patient cannot link their account.

Check that the patient’s first name, surname, date of birth and NHI or Medicare number in CCCare are correct and match what they entered in the app. Confirm they can access the email address held in CCCare, and ask them to check their junk or spam folder for the linking code. Correct any outdated details and ask the patient to try again. If linking still fails, contact IBD Connect Support at CCCure [ibd-connect@c-c-cure.org].

A patient says they haven't received their questionnaire.

Ask the patient to check for the orange questionnaire card under To do on the app’s Home screen. For the email notification, they should check the email address held in CCCare, including their junk or spam folder. In the Questionnaires tab in CCCare, confirm that:

- Their app account is successfully linked.

- The first app questionnaire has been sent manually.

- There are no outstanding patient portal questionnaires that need to be reviewed, completed or cancelled.

A patient wants to stop using IBD Connect.

Ask why they wish to stop and whether they need help using the app. Share their feedback with IBD Connect Support at CCCure so any issues can be addressed. Encourage continued use where possible, as the patient portal will be phased out in the near future. If a temporary return to the portal is necessary, open Questionnaires in their CCCare record, cancel any pending app questionnaires, then send a new questionnaire from the Patient Portal section.
